Continued Service Agreement
(CSA)
Applicable to Civilian Employees only! CSA is required if cost of training is $5,000 or more and/or training is 80 hours or more.

Use this form to request training for MRTP courses. Only EDS & UTMs authorized to utilize this form.
AFDW
TDY-to-School Worksheet
If you have an AETC Centrally funded course seat (AT, CT, OT quota type), you are required to complete this form.
AFDW
Individual Development Plan
5 CFR 412.401, DoDI 1400.25, Volume 410, AFI 36-2670, and AFMAN 36-606 requires mandatory completion (civilians).
Sister Service Course Student
Enrollment (Type 5 Form)
Use this form to request sister service (Army, Navy, etc…) MRTP courses. Only EDS & UTMs authorized to use.
